Job Description: DaVita is a rapidly growing Fortune 500 Healthcare Services company and a leading provider of kidney care in the United States. The goal of this role is to ensure DaVita is the provider of choice to hospitals. This teammate will engage renal discharge planners and case managers in an effort to build and strengthen relationships. Their focus is to ensure a smooth and seamless experience in support of newly diagnosed and hospitalized dialysis patients to ensure a successful transition to the outpatient setting.
